Fitness Liability Waivers

Waivers that lapse on a date, with guardian and medical-clearance gates.

The short answer

Fitness Liability Waivers is an Odoo app for gyms, studios and sports clubs whose cover currently lives in a folder of signed PDFs. Each waiver computes and stores an expiry date from its template's term, so cover lapses by itself on the right morning. A minor's waiver needs a guardian signature, and a flagged readiness questionnaire waits on clearance a manager records.

What it is for

The waiver you are relying on expired months ago and nobody noticed, because a signed PDF in a folder has no expiry date. The member has been training on it ever since, and the document that was supposed to protect the club protects nothing, which everyone finds out at the same moment. The other half of the problem is the minor whose parent never signed, and the member who ticked boxes on a readiness questionnaire and was never sent for clearance. All three are the same failure: paper that is filed rather than a record with a state.

What is inside it

    Expiry

    The expiry date is computed from the signed date and the template's term, then stored, and expiry is evaluated against today. Cover lapses on its own, with nobody running anything.

  2. 02

    Minors and PAR-Q

    A waiver for a minor carries the guardian's name, relationship and their own signature, and is not in force without them. A flagged questionnaire waits on a separate clearance field.

    Templates

    The wording a member agrees to and the term that decides when it lapses sit on one record. Changing the term changes future waivers and leaves signed ones alone.

    The board

    Six tiles: expired, awaiting clearance, expiring inside thirty days, coverage rate, minors and members on file. Clicking one opens the members it counted.

What it looks like in practice

  1. Two members cannot train today and three more are close behind

    Cover has already lapsed for two members on the book. Three others are still covered but their expiry date falls inside the month.

    Expiring and expired are separate counts on the board, deliberately, because an expired waiver is somebody you stop at the desk and an expiring one is an email you can send this week. Each tile opens the members behind it.

  2. A minor signs and the waiver still is not in force

    A member record is marked as a minor, and a waiver gets signed against a template at the front desk.

    The guardian requirement is set from the member, so the waiver carries the guardian's name, relationship and signature, and without them it does not count as in force. That is the correct legal reading and the one a club is least likely to enforce by hand.

  3. The house wording changes and signed waivers keep their old term

    A club rewrites its waiver text and shortens the validity period it wants going forward.

    The edit lands on one template record. Waivers signed later compute expiry from the new term, and waivers already signed keep theirs, because a member agreed to the document that was in front of them.

What it deliberately does not do

No e-signature service
A signature is captured and stored with the signer's name and IP. It is not routed through DocuSign and carries no certificate.
No membership billing
Members are records that hold waivers. Subscriptions, dues and door access belong to another model.
No class booking
Nothing here schedules a session or checks a member into one.
No jurisdiction library
The template text is yours to write. This enforces the term and the signatures, not whether the wording holds up where you trade.

Questions

Can I install this on Odoo Online?
No. Odoo Online cannot install modules that contain Python, so this runs on Odoo.sh or a self-hosted instance. Check the versions listed above for the series and edition it is verified against.
Is the signature legally certified?
It is captured and stored with the signer's name and IP address, and that is all. Nothing routes through an e-signature service and no certificate is attached, so if your insurer or your lawyer wants a certified audit trail you need a signing service beside this.
Does it manage memberships, dues or class bookings?
No. This is the paperwork layer. A member record exists to hold waivers and their state, and everything commercial or operational around it, from dues to door access to the timetable, stays in whatever system already runs it.
Will the template text cover my jurisdiction?
No, and it does not try. The wording is yours to write, and what gets enforced is the term and the signatures each waiver requires. Have the text reviewed locally before you start collecting signatures against it, because nothing here validates the clauses.
