Clinical Charting
SOAP charting where signing locks the note and a correction becomes an addendum.
The short answer
Clinical Charting is an Odoo module for therapy, wellness and allied-health practices that write SOAP notes. Subjective, objective, assessment and plan are separate fields; signing a chart locks them and stamps who signed and when, so any later correction has to arrive as a dated addendum. A clickable musculoskeletal body map carries the problem list, and a board counts what is still unsigned.
What it is for
An unsigned note is a draft that looks like a record. It reads the same in the file, says the same things, and carries none of the weight, because nobody has attested to it. Practices find this out in a batch. Someone runs a report at month end, finds a pile of open notes, and the clinicians who wrote them are reconstructing visits from memory. What would have prevented it is small and boring: how many are open right now, and how old the oldest one is.

Signing
A chart moves from Draft to Signed once. Signing stamps the signer and the moment as readonly fields, freezes the four SOAP fields, and leaves Add Addendum as the only way back in.

The body map
Two anatomical plates behind a Front and Back toggle, with thirty clickable zones from deltoid to erector spinae. Selecting a zone lists every problem on it; charting a new one is a button.

Templates
Charts come in four types, and a template seeds all four fields for the kind of visit being written. Templates are records, so changing the house wording for intake is an edit.

Six tiles. Signed rate reports how the practice is doing, the count past 72 hours reports what to do today, and clicking a tile opens the charts it counted.

The visit happened and the SOAP fields were typed, but nobody pressed Sign. Nothing on the chart says how long it has been sitting there.
Turnaround is computed from the service datetime to the signature and stored on the chart, so an open note is measurable rather than merely open. The delinquent tile counts anything past 72 hours and opens those charts.

Years of problems arrive as a list nobody can read at a glance
A patient's history is ordered by date, and the clinician wants to know what is currently active on one leg.
Each problem carries a muscle or spinal segment, a side, an onset date and a status. The Front and Back plates paint the charted zones, each side of the toggle keeps its own count, and selecting a zone lists what sits on it.

The whole visit gets typed into the first box on the form
Subjective is where the cursor lands, so the assessment ends up buried mid-paragraph and a colleague reading it back six months later has to hunt for it.
Picking a note template lands prepared wording in all four fields, so each of subjective, objective, assessment and plan starts with something in it. Four chart types cover SOAP, progress, intake and discharge.

What it deliberately does not do
- No coding and no claims
- Problems carry a name, a region and a status. There is no ICD-10 field and nothing bills.
- No e-signature certificate
- Signing records the Odoo user and the moment. It is not a cryptographic signature and does not claim to be one.
- No HL7 or FHIR exchange
- Charts stay in Odoo. Nothing here speaks to an external EMR.
- No scheduling
- A chart references a patient and a practitioner. Booking the visit is somebody else's model.
